灯盏花素治疗颈内动脉系统脑梗死的效果及对脑循环动力学的影响

摘    要:目的观察和研究灯盏花素(BC)治疗颈内动脉系统动脉硬化性脑梗死(ATCI)患者的疗效及对脑循环动力学的影响。方法采用随机、双盲和平行对照的方法,把患者分为两组,同样接受脑梗死的基本治疗。而灯盏花素组病例,每天1次,加用灯盏花素20mg入10%葡萄糖液500ml中静脉滴注,连用10d。对患者进行用灯盏花素治疗前、治疗中、治疗后的脑神经功能缺损评分,同时测定脑循环动力学参数(CVDP)。结果譹灯盏花素组神经功能缺失,治疗有效率为94%,对照组为60%(P<0.05)。譺灯盏花素组患者病灶一侧CVDP中,脑血流量(Q)及血流速度(V)用灯盏花素治疗中及治疗后较治疗前显著增加(P<0.05),较对照组亦有显著增加(P<0.05)。血管阻力(R)、弹性及临界压力(CP)经灯盏花素治疗后,较治疗前明显降低(P<0.05),较病灶对侧大脑亦差异有显著性(P<0.05)。结论BC注射液治疗ATCI患者疗效好,能明显改善脑血流灌注,对缺血性脑组织有一定保护作用,使部分CVDP趋向正常。

Abstract:Objective To observe and study the clinical efficacy of breviscapine (BC) and its effect on cerebral hemodynamics in patients with atherothrombotic cerebral infarction (ATCI) of the carotis interna arteria system. Methods This clinical trial was randomly double-blind and parallel controlled. The patients with ATCI were divided into two groups. All patients recieved same basic treatment; but the BC was only added to the trial group. Signaturei BC 20 mg+10% glucoseinj 500 ml, intravenous drip once daily for consecutive 10 days, then scoring the condition of neural functional deficits before, during and after the treatment; at the same time the cerebral hemodynamic parameters (CHDP) of the patients were measured. Results {1}The neural functional deficits of patients in trial group tended to be repaired. The effective rate (ER) was 94%, but the control group′s ER was 60%. There was marked statistical significantce between the two groups (P<0.05). {2}The total cerebral blood flow and velocity (V) in the focal side of brain during and after the treatment in the BC group showed a statistically significant increase, compared with those before the treatment (P<0.05), and even with the control group (P<0.05). And the cerebral vascular resistance (R), elasticity and critical pressure (CP) showned a statistically significant decrease (P<0.05). Conclusion These results revealed that the BC is effective in relieving the neural functional deficits of patients with ATCI. Breviscapine is able to normalize parts of the CVDP, protecting the brain.
