| 摘 要： | 在糖尿病相关性抗体中，胰岛素自身抗体（IAA）是唯一仅识别β细胞特异性抗原的抗体，对预测1型糖尿病（TIDM）的发生有较高的价值。然而，与胰岛素结合的抗体包括糖尿病相关性IAA、糖尿病非相关性IAA和胰岛素抗体（IA）。传统的血清学研究显示糖尿病相关性与非相关性IAA识别胰岛素表位存在差异；采用重组单克隆抗体技术能有效分析IAA或IA的识别表位；应用噬菌体展示技术能成功地区分糖尿病相关性IAA与IA的独特型。这些检测IAA的新技术为自身免疫性糖尿病的预测和发病学研究提供了新手段。

| Abstract: | Several markers for type 1(autoimmune) diabetes have been identified, but only insulin autoantibodies (IAA) binds with an antigen specific to the beta cell and IAA has a high predictive value on the development of diabetes. However, antibodies that bind with insulin involve diabetes-related IAA, diabetes-unrelated IAA and insulin antibodies (IA). Conventional serological studies have shown epitope restriction of IAA can differentiate diabetes-related IAA from unrelated IAA. Recombinant monoclonal antibodies or their Fab as reagents prove to be effective in the epitope analysis of IAA or IA; phage display can distinguish idiotypes of diabetes-related IAA from IA. These new technologies may improve our understanding of the underlying autoimmune response and aid in the accurate prediction of type 1 diabetes. |
