急性心肌梗死患者与负性情绪的相关因素探讨

目的:为探寻负性情绪与急性心肌梗死(Acute Myocardial Infarction,简AMI)的病因相关因素。方法:对91例AMI患者和90例正常人应用SCL-90,LES,SSRS,HAMD,HAMA及一般情况调查表进行对照调查。结果:与正常人对照:AMI组发作前有明显心理刺激因素的有73例(80.22%)。心理健康状态方面:AMI组SCL-90的总分、阳性项目分、躯体化、强迫、抑郁、焦虑、敌对、恐怖、人际、偏执等因子分均显著高于对照组(P<0.01),提示AMI患者存在更多的心理障碍,心理健康状态较差。AMI组负性生活事件分更高(P<0.01),社会支持度更低(P<0.01)。负性情绪方面:AMI组在焦虑/躯体化、认知障碍、阻滞、绝望、睡眠障碍和焦虑抑郁总分均明显高于对照组(P<0.01)。结论:负性生活事件多发,缺乏亲人和社会支持,较差的心理健康状态和更多的负性情绪可能是急性心肌梗死发生发展的危险因素。

A Study of Related Factors Between Negative Emotion and Acute Myocardial Infarction

Objective:To study etiological relation between negative emotion and acute myocardial infarct(AMI).Methods:91AMI patients and 90 normal persons were investigated with SCL-90,LES,SSRS,HAMD,HAMA and generally investigated tables.Results: Compared with normal group.AMI group had 73 cases(80.22%) that had obvious mental stimulated factors before they attack heart disease;In mental health condition aspect:AMI patients got higher score in anxiety,depression,obsession,interpersonal,sensitivity,phobia,paranoid,somatization(P<0.01),It showed AMI pations had much more mental disorder and more worsen mental health condition.AMI group had clearly higher scores of negative life events in a year before and lower scores in social supports(P<0.01).In negative emotion aspect:AMI group had clearly higher scores in somatization disorder of anxiety and depression,cognitive disorder,retardation,desperation,sleeping disorder,sum scores of anxiety and depression(P< 0.01).Conclusions: more negative life events,laking relatives and social supports,worsen mental health condition and more negative emotion may be high rish factors of AMI and make patients' condition worsen.
