苯磺酸左旋氨氯地平对糖尿病合并高血压患者血清胱抑素C、尿酸及血管内皮功能的影响探究

摘    要:目的 探讨苯磺酸左旋氨氯地平对糖尿病合并高血压患者血清胱抑素C(Cys C)、尿酸(UA)及血管内皮功能的影响。方法 选取成都医学院第一附属医院及重庆市中医院/重庆市中医研究院2016年3月至2018年9月收治的160例糖尿病合并高血压患者为研究对象,随机分为观察组和对照组,每组各80例,对照组患者给予常规降糖与降压(贝那普利)治疗,观察组在此基础上加用苯磺酸左旋氨氯地平,两组疗程均持续4周。比较两组患者干预前后血压水平、血清Cys C、UA水平及血管内皮功能的变化。结果 两组患者干预前的基线资料、收缩压(SBP)、舒张压(DBP)、血清Cys C、UA、一氧化氮(NO)及内皮素(ET-1)水平比较,差异无统计学意义(P>0.05),具有可比性。两组患者干预后的SBP、DBP、血清Cys C、UA、ET-1水平较干预前均显著降低,血清NO水平较干预前升高,差异有统计学意义(P<0.05)。观察组干预后SBP、DBP、Cys C、UA、ET-1均低于同期对照组,NO高于同期对照组,差异有统计学意义(P<0.05)。结论 在常规降糖、降压治疗方案的基础上加用苯磺酸左旋氨氯地平能进一步控制糖尿病合并高血压患者的血压水平,降低血清Cys C、UA水平,改善血管内皮功能。

Abstract:Objective To investigate the effect of levamlodipine besylate on serum cystatin C(Cys C),uric acid(UA)and vascular endothelial function in diabetic patients with hypertension.Methods 160 patients with diabetes mellitus and hypertension admitted to the hospitals from March 2016 to September 2018 were enrolled for the study and randomly divided into observation group and control group,80 cases in each group.Patients in control group were treated with antidiabetic and antihypertensive(Benazepril)treatment.On this basis,cases in the observation group were given levamlodipine besylate for 4 weeks.The changes of blood pressure,serum Cys C,UA levels and vascular endothelial function were compared between two groups before and after intervention.Results There were no significant differences in baseline data,systolic blood pressure(SBP),diastolic blood pressure(DBP),serum levels of Cys C,UA,nitric oxide(NO)and endothelin(ET-1)before intervention between two groups(P>0.05).SBP,DBP,FPG,serum levels of Cys C,UA and ET-1 after intervention in two groups were significantly lower than those before intervention,and serum level of NO was significantly higher than that before intervention,and the differences were statistically significant(P<0.05).SBP,DBP,FPG,serum levels of Cys C,UA and ET-1 after intervention in the observation group were lower than those in the control group at the same time,and serum level of NO was higher than that in control group at the same time(P<0.05).Conclusion On the basis of antidiabetic and antihypertensive treatment,levamlodipine besylate could further control blood pressure,reduce serum Cys C and UA levels and improve vascular endothelial function in patients with diabetes mellitus and hypertension.
