重症肌无力患者外周血中CD4~+CD25~+调节性T细胞的变化及意义

研究CD4 + CD2 5 + 调节性T细胞在重症肌无力 (MG )发病中的作用。本文采用三色流式细胞术对 2 9例MG患者和 2 3例健康对照者外周血中CD4 + CD2 5 + T细胞 (CD3+ CD4 + CD2 5 + )的百分率进行测定。结果显示病情未能很好控制的MG患者外周血CD4 + CD2 5 + T细胞比率略低于健康对照组 (分别为 3 79%± 1 4 0 %、 4 5 3%± 0 96 % ,P =0 12 ) ,病情稳定或缓解的MG患者CD4 + CD2 5 + T细胞比率 (8 4 5 %± 1 96 % )显著高于健康对照组 (P =0 0 0 0 1) ;胸腺切除的MG患者CD4 + CD2 5 + T细胞比率 (8 4 4 %± 2 39% )显著高于非胸腺切除的MG患者 (5 88%± 2 89% ,P =0 0 38)和健康对照组 (4 5 3%± 0 96 % ,P =0 0 0 3)。提示MG患者外周血中存在异常比例的CD4 + CD2 5 + 调节性T细胞 ,可能参与疾病的发生与发展。

Changes of CD4 + CD25 + T Cell Frequencies in the Peripheral Blood of Patients with Myasthenia Gravis

To investigate whether there are differences in peripheral blood CD4+CD25+T cell frequencies between MG patients and healthy controls,the peripheral blood CD4+CD25+T lymphocytes of 29 MG patients and 23 healthy controls were detected by three-color FACS analysis. It was found that myasthenic patients with symptomatically uncontrollable disease (n=12) showed a slightly lower percentage of CD4+CD25+T cells (mean % 3.79±1.40) compared with those of healthy controls (mean % 4.53±0.96,P=0.12),while patients with clinically stable disease (n=17) showed a significantly increased level of CD4+CD25+T cell (mean % 8.45±1.96,P=0.0001). In addition,thymectomized MG patients (n=7) had significantly higher percentage of CD4+CD25+T cells (mean % 8.44±2.39) compared with both nonthymectomized MG patients (n=22,mean % 5.88±2.89,P=0.038) and healthy controls(P=0.003). These observations indicate the existence of abnormal proportions of CD4+CD25+T cells in MG patients which may correlate with disease activity.
