介入封堵术治疗先天性心脏病213例疗效评价

目的：观察介入封堵治疗先天性心脏病的近期临床效果,总结相关临床经验。方法：回顾分析自2005年6月至2010年9月期间我院实施介入治疗的先天性心脏病患者213例的资料,男98例,女115例;年龄2～68岁,其中动脉导管未闭（PDA）78例,室间隔缺损（VSD）38例,房间隔缺损（ASD）82例,肺动脉瓣狭窄（PS）15例;手术采用经皮穿刺股静脉和/或股动脉的方法,术前和术后进行左、右导管造影检查。结果：技术成功率达98.59%（210/213）,并发症发生率为1.41%（3/213）,封堵成功患者术后复查随访,原有杂音均消失或者明显减弱,经胸超声心动图检查未发现明显分流。结论：利用介入封堵术治疗先天性心脏病具有成功率高、创伤小、并发症少,死亡率低的特点,开辟了先天性心脏病治疗的新领域。

Clinical assessment of interventional closure in 213 cases with congenital heart disease

Objective: To observe short term clinical effects of interventional closure in treatment of congenital heart disease(CHD) and summarize relative clinical experience.Methods:Retrospective analysis was performed on 213 CHD patients undergoing interventional therapy in our hospital from Jun 2005 to Sep 2010.There were 98 males and 115 females with age 2~68 years,including 78 cases with patent ductus arteriosus(PDA),38 cases with ventricular septal defect(VSD),82 cases with atrial septal defect(ASD) and 15 cases with pulmonary stenosis(PS).All the interventions were performed via percutaneous puncture of femoral vein and/or femoral artery,and left and right catheter angiography were conducted in all patients before and after operation.Results:Technical success rate was 98.59%(210/213),and incidence rate of complications was 1.41%(3/213).During follow-up of patients after successful closure,original noise disappeared or significantly weakened,and there was no obvious shunt by transthoracic echocardiography in all patients.Conclusion: Interventional closure for congenital heart disease possesses high success rate,less injury,fewer complications,low mortality rate.It opens a new area for treatment of congenital heart disease.
