成软骨细胞瘤临床分析

目的总结肢体成软骨细胞瘤的手术治疗及疗效。方法总结同济大学附属同济医院骨科1991年2月至2008年2月经手术治疗的肢体成软骨细胞瘤患者11例,其中男9例,女2例;年龄12-38岁,平均20.14岁。肿瘤部位：股骨远端5例,胫骨近端5例,肱骨近端1例。手术方法均采用病损局部开窗切刮术,术后腔内用石炭酸、75%酒精灭活。病损切刮术后骨缺损填充采用自体骨6例,异体骨1例,人工骨1例,骨水泥3例。未作内固定。结果11例均获随访,随访时间3个月至16年,平均8.6年。8例患者病损区植骨均愈合,患肢功能恢复良好,3例复发,其中1例恶变。结论成软骨细胞瘤有一定的复发率和恶变率,初次手术的彻底性影响患者预后。

Clinical analysis of chondroblastoma

Objective To evaluate the surgical effect of treating chondroblastoma in extremities. Methods Summarize 11 cases with chondroblastoma in extremities treated with surgery in Tongji Hosptal, Tongji University from Feb. 1991 to Feb. 2008. There were 9 males,2 females,with an average age of 20. 4 years （ ranging from 12 to 38 years）. Chondroblastoma was located at the distal femur in 5 cases, the proximal tibia in 5 cases and the proximal femur in 1 case. All patients underwent curettage, combined with cauterization of cyst-infested interior wall by using phenol and ethanol. Bone defect were filled with autogenous bone （7 cases）, decalcified allogenous bone （ 1 case）, artificial bone （ 1 case）, or bone cement （ l case）. No patient was treated with internal fixation. Results An average 8.6 years＇ follow-up （ ranging from 3 months to 16 years） was carried out in all 11 cases. In 8 cases bone graft of lesion eventually healed with satisfactory function of the affected extremities, with 3 recurrence and 1 malignant transformation. Conclusion Chondroblastomas are bound to have certain rates of recurrence and malignant transformation. The thoroughness of the initial operation directly impacts the prognosis of patients with chondroblastoma.
