元宫药铜220宫内节育器的系统评估

摘    要:目的系统评价元宫药铜220宫内节育器(IUD)的有效性、副反应和可接受性。方法电子及手工检索1994年1月~2006年7月国内外发表的相关文献,并依照循证医学的方法对纳入评估的文献进行筛选、评价和分析。结果共检索到相关文献13篇,4篇纳入本系统评估。评估结果显示,置器12个月内元宫药铜220IUD与宫铜300IUD临床使用效果较为接近;置器24个月内元宫药铜220IUD临床放置效果与γ药铜200基本接近,趋向优于TCu220C、母体乐375及宫药铜200等IUD。结论元宫药铜200短期随访的有效性、可接受性较好,但因缺少资料而难以对其长期使用效果进行评价。建议在大规模推广之前就元宫药铜220IUD的有效性、安全性开展设计严密的随机对照临床试验,观察年限不少于5年。

Abstract:Objective: To systemically evaluate the safety, effectiveness and acceptability of medicated YGCu220 IUD. Methods: We identified relevant literatures published during January 1994 and July 2006 by using electronic and hand search. Included papers were systematically selected and evaluated according to evidence based medicine. Results: A total of 13 relevant papers were identified, and 4 papers were selected. The one-year clinical performance of medicated YGCu220 was similar with that of uterine cavity-shaped Cu300, and the two-year performance was similar with that of medicated γCu 200 but better than that of TCu220C, MLCu375, or medicated uterine cavity-shaped Cu200. Conclusion: The one-or two-year effectiveness and acceptability of medicated YGCu220 are good. However it is hard to evaluate its long-term clinical performance because of data limitation. The suggestion is that the randomized clinical trials (RCTs) with strict design should be conducted before the expansion use of YGCu220, and the follow-up survey should be continued at least five years.
