2型糖尿病合并亚临床甲减患者血脂及颈动脉内膜中层厚度的变化

目的:探讨2型糖尿病(T2DM)合并亚临床甲状腺功能减退(SCH)患者血脂谱及颈动脉内膜中层厚度(CIMT)的改变.方法:选择2型糖尿病并甲状腺功能正常(EUT)40例患者为T2DM+EUT组,2型糖尿病合并亚临床甲减患者38例为T2DM+SCH组,比较两组患者的血脂谱及颈动脉内膜中层厚度.结果:T2DM+SCH组与T2DM+EUT组比较,低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)增高而高密度脂蛋白胆固醇(HDL-C)降低,CIMT增厚,差异均有统计学意义(P＜0.05).LDL-C、TC及CIMT与促甲状腺激素(TSH)呈正线性相关.结论:SCH可加重T2DM患者血脂谱的紊乱程度,T2DM合并SCH促使CIMT明显增厚,由此增加糖尿病患者发生动脉粥样硬化及血栓性疾病的风险.

Changes of Serum Lipid Profile and Carotid Intima-media Thickness in Type 2 Diabetic Patients with Subclinical Hypothyroidism

Objective：To investigate the changes of lipid profile and carotid intima - media thickness （CIMT） in type 2 diabetes mellitus （ T2DM ） with subclinical hypothyroidism （SCH）. Methods： Serum total cholesterol （TC）, total triglycerides （TG）, low density lipoprotein - cholesterol （LDL - C ）, high density lipo- proteincholesterol （HDL- C） and CIMT were compared between the 38 patients with T2DM combined with SCH and the 40 patients with T2DM combined with euthyroidism （EUT）. Results：TC, LDL - C and CIMT were significantly higher in T2DM ＋ SCH group than those in T2DM ＋ EUTgroup. TC, LDL - C and CIMT were linearly and positively correlated with thyroid -stimulating hormone （TSH） level. Condusions：SCH may aggravate the disturbance of lipid profile and increase the level of CIMT, which will increase the risk factor for the development of atheroselerosis and cardiovascular occlusive diseases in the patients with T2DM.
