动态增强CT扫描在诊断肝癌中的应用

目的 探讨动态增强CT扫描诊断肝癌的应用价值。方法 120例肝癌经动态增强CT扫描诊断并获证实。其中45例为单层动态增强扫描,75例为移动式动态增强扫描。单层动态增强扫描为选择平扫病变最显著的一层作连续重复扫描,移动式动态增强扫描为注射造影剂完毕后对全部肝脏进行扫描,两种扫描方式均做选定层面的延时扫描。结果 两种扫描方式显示血供丰富的肝癌在肝动脉期增强,与正常肝组织比较显示为高密度图像,在门静脉增强期,同一病变区显示为低密度图像;单层动态扫描45例,在肝动脉期病灶增强率82％;移动式动态扫描75例,在肝动脉期病灶增强率95％。并且增强扫描能显示肿瘤供血情况、肿瘤包膜、肝内动静脉短路、肝门脉和下腔静脉肿瘤栓子等征象。结论 动态增强CT扫描能够提供更多的诊断信息,是一项有价值的诊断方法。

Application of Dynamic Enhancement CT Scanning in Diagnosis of Hepatic Carcinoma

Objective To assess the value of dynamic enhancement scanning on diagnosis of hepatic carcinoma. Methods 120 cases were studied and recognized by dynamic enhancement scanning,45 cases with single level dynamic scan and 75 cases with incremental dynamic scan the former is applied by selecting an image with obvious lesion and the latter is applied by scanning the whole liver after injection of contrast media, delay scanning was all done in two scanning methods. Results Hie images of two scanning methods ware as follows: the speciality of enhancement was relative to the double blood supply in the liver. A typical hypervascular hepatic carcinoma was enhanced during hepatic artery period, its images showed hyperdense compared to the normal liver period.The enhancement rate of 45 cases with single level dynamic scan was 82 percent;The enhancement rate of 75 cases with incremental dynamic scan was 95 percent. The images of enhancement could show other characters of hepatic carcinoma, including tumor vascularity, tumor capsule,arteriovenous shunt,tumor thrombus in the portal vein and inferior vena cava,especially, dynamic enhancement CT scanning was useful in diagnosis of equal density hepatic carcinoma and small tumors which was 3 ~ 5cm in diameter. Conclusion Dynamic enhancement CT scanning could give more evidences in diagnosis of hepatic carcinoma. It's a valuable diagnostic method.
