完全腹腔镜下脾肿瘤切除术的临床体会

目的探索完全腹腔镜技术在脾肿瘤切除术中的优缺点和安全性。方法将2008年7月至2015年11月之间收治的58例脾肿瘤,随机分成完全腹腔镜组和开腹组予以手术治疗,观察各项临床指标,并对结果进行比较分析。结果完全腹腔镜组住院时间为(9.2±1.2)d,术中出血量为(129.7±38.0)ml,手术切口长度为(8.3±1.1)cm,术后病人疼痛评分为(2.7±0.7)分;开腹组住院时间为(10.6±1.2)d,术中出血量为(155.9±59.0)ml,手术切口长度为(17.2±1.5)cm,术后病人疼痛评分为(4.3±0.8)分。完全腹腔镜组以上指标上均优于开腹组,差异均有统计学意义(均P0.05)。在手术时间和术后并发症指标上两组间比较,差异无统计学意义(P0.05)。结论在脾肿瘤手术治疗中,完全腹腔镜下脾肿瘤切除是一种安全、有效的手术。

Clinical experience of completely laparoscopic splenic tumor resection

Objective To explore advantages and disadvantages or security in totally laparoscopic technique for spleen tumor resection.Methyds Fifty-eight cases of splenic tumor from July 2008 to November 2015 were collected,and randomly divided into complete laparoscopy group and open surgery group.The clinical indicators,and the outcomes were compared.Results In the totally laparoscopic group,hospital time was 9.2 ± 1.2 days,blood loss was 129.7 ± 38.0 mL,incision length was 8.3 ± 1.1 cm and postoperative pain score was 2.7 ± 0.7,and those in the open surgery group was 10.6 ± 1.2 days,155.9 ± 59.0 mL,17.2 ± 1.5 cm and 4.3 ± 0.8,respectively (P＜0.05 for all).But there was no significant difference in operating time and postoperative complications between two groups (P ＞0.05).Conclusions In the spleen tumor surgery,completely laparoscopic resection is a safe and effective operation.
