原发性非小细胞肺癌体外化疗药物敏感试验研究的临床价值

摘    要:目的 研究非小细胞肺癌对化疗药物体外敏感程度,提高临床选药的科学性。方法 采用MTT比色法对34例非小细胞肺癌新鲜手术标本进行14种常用化疗药物的敏感性测定。结果 肺癌标本对化疗药物敏感程度(中位抑制率)依次为顺铂80.69%、异环磷酰胺79.50%、丝裂霉素79.19%、卡铂77.84%、柔红霉素74.12%、氟尿嘧啶72.88%、羟基喜树碱72.35%、平阳霉素67.60%、表阿霉素66.95%、阿霉素65.65%、阿糖胞苷37.32%、环磷酰胺35.89%、甲氨蝶吟35.66%、依托泊苷23.64%;其中顺铂、异环磷酰胺、卡铂对所有肿瘤标本均示敏感,而阿糖胞苷、甲氨蝶呤、环磷酰胺、依托泊苷对半数标本耐药。结论 体外化疗敏感试验在非小细胞肺癌对化疗药物敏感性预测中具有可行性和必要性,对个体化化疗方案的确定具有一定的临床价值。

Abstract:Objective To investigate the in vitro sensitivity of primary n on-small cell lung cancer(NSCLC) to chemotherapy drugs in order to improve the scientific selection of medicine.Methods The sensitivity in vitr o of 14 commonly-used chemotherapy drugs was detected in fresh surgical samples of 34 NSCLC using MTT colorimetric assay.Results The chemosensi tivity of 14 drugs was as follows: DDP 80.69%,IFO 79.50%,MMC 79.19%,CARP 77.84%, DRN 74.12%,5-Fu 72.88%,HCPT 72.35%,BLEO 67.60%,EPI 66.95%,ADM 65.65%,Ara-C 3 7.32% ,CTX 35.89%,MTX 35.56%,VP-16 23.64%.DPP,IFO and CARP were sensitive to a ll tumor samples,while Ara-C,MTX,CTX and VP-16 presented drug resistance to ha lf samples.Conclusion The in vitro chemosensitivity is feasible and necessary to predict the sensitivity of NSCLC and has clinical value to dete rmine the individualized chemotherapy.
