1起学校甲型H1N1流感混合季节性流感暴发的调查

目的]阐明1起学校甲型H1N1流感混合季节性流感暴发疫情的特征,评价综合防控措施。方法]通过现场流行病学调查和实验室检测进行综合分析。结果]疫情历时9d,共报告流感样病例85例,罹患率6.66%;在发生首发病例2d后发病达到高峰,采取控制措施后发病逐步下降;寄宿生罹患率(9.92%)高于走读生(3.72%),差异有统计学意义(P<0.01,OR为2.85,95%CI为1.763～4.606);对30例流感样病例采集咽拭标本,21例甲型H1N1流感核酸阳性,9例季节性流感核酸阳性。结论]这是1起学校甲型H1N1流感混合季节性流感暴发;落实学校传染病疫情报告和晨检制度是疫情及时控制的关键。采取停课是控制疫情扩散,及早扑灭疫情的有效措施。

Survey on an Outbreak of Influenza A(H1N1) Mixed with Seasonal Influenza in a Junior High School

Objective]To clarify the features of an outbreak of influenza A(H1N1) mixed with seasonal influenza,so as to evaluate the comprehensive control measures.Methods]The epidemic was analyzed through field epidemiological investigation and laboratory testing.Results]A total of 85 influenza-like illness(ILI)cases were found during the epidemic which lasted for 9 days with the attack rate of 6.66%;the incidence peaked on third day,and then decreased after effective measures were performed;the attack rate of resident students(9.92%)was higher than that of day students(3.72%)(P<0.01,OR:2.85,95%CI:1.763-4.606);In 30 throat swabs of ILI cases,21 throat swabs were positive for Influenza A(H1N1) virus nucleic acid,9 throat swabs were positive for seasonal virus nucleic acid.Conclusion]This is an outbreak of influenza A(H1N1) mixed with seasonal influenza in a junior high school.Implementing the systems of school infectious disease notification and morning-checking are the keys to controlling the epidemic timely.Adopting class suspension is the effective measure of putting out the epidemic early.
