风湿宁胶囊治疗类风湿关节炎机制的研究

摘    要:目的观察风湿宁胶囊(FSN)对类风湿关节炎(风寒湿痹型)新西兰大白兔血清和关节滑液中白细胞介素IL-6、IL-17、IL-4、IL-10水平的影响,探讨其治疗类风湿关节炎(rheumatoid arthritis,RA)的可能机制。方法雄性新西兰大白兔120只随机分为正常组、模型组、甲氨蝶呤(methotrexate,MTX)组、风湿宁胶囊低剂量组、风湿宁胶囊中剂量组、风湿宁胶囊高剂量组,每组20只。除正常组外,其他组均制备RA风寒湿痹动物模型。治疗组分别给予风湿宁胶囊溶液、甲氨蝶呤悬浮液,正常对照组灌生理盐水。用药7天后用酶联免疫法测定各组血清及关节滑液中IL-6、IL-17、IL-4、IL-10的水平。结果FSN中、高剂量组和MTX治疗组血清及关节滑液的IL-6、IL-17较模型组明显降低(P0.05),而FSN低剂量组IL-6、IL-17水平的下降不明显(P0.05)。FSN各剂量组和MTX组中IL-4、IL-10水平较模型组均升高,但差异无统计学意义(P0.05)。结论风湿宁胶囊不仅能下调新西兰RA大白兔血清及关节滑液中过高的IL-6、IL-17水平,而且能上调血清及关节滑液中IL-4、IL-10的水平,提示风湿宁胶囊治疗RA的机制可能是通过平衡促炎因子和抗炎因子的效应进而散寒通络,降低关节滑膜炎症,缓解临床症状。

Abstract:Objective To observe the effect of Fengshining capsule (FSN) on interleukin (IL)-6,IL-17,IL-4,IL-10 in serum and joint synovial fluid of CIA model rats with wind-cold-dampness arthralgia,and further to investigate the possible mechanism for the treatment of rheumatoid arthritis.Methods New Zealand male white rabbits were randomly divided into normal control group,model control group,methotrexate group,low-dose of FSN group,middle-dose of FSN group and high-dose of FSN group (n=20 each).The CIA model with wind cold dampness syndrome was established in all the groups except for the normal control group.The treatment group was given FSN solution and MTX solution respectively.The control group was given normal saline.The levels of IL-6,IL-17,IL-4 and IL-10 in the serum and joint synovial fluid of rabbits were detected by ELISA after seven days'treatment.Results Comparing with the model control group,the levels of IL-6 and IL-17 significantly reduced in middle-dose and high-dose of FSN groups and methotrexate group(P<0.05),but the levels of IL-6 and IL-17 in low-dose of FSN group decreased not obviously (P>0.05).Comparing with the model control group,the levels of IL-4 and IL-10 in treatment groups all rised,but there was no statistic difference.Conclusion The FSN can not only reduce the levels of IL-6 and IL-17,but also can increase the levels of IL-4 and IL-10 in the serum and synovial fluid of RA rabbits,suggest the mechanism of FSN may by means of balancing the effect of proinflammatory factor and anti-inflammatory cytokines,expelling cold and dampness to activate collaterals,reducing joint synovial inflammation and relieving clinical symptoms.
