显微接管术修复43例外伤性小泪小管断裂的疗效分析

目的：对43例采用显微接管术修复的急性或陈旧性外伤性下泪小管断裂的病例进行疗效分析。方法：在手术显微镜下，从伤口处寻找下泪小管的鼻侧断端，将硅胶条经泪小管插至鼻腔中，修复泪小管。结果：43例下泪小管断裂患者进行显微接管修复术的成功率为90．7％，其中4例因术后硅胶条脱落，进行其他修复术。结论：对于急性病例应早期进行手术修复。陈旧性下泪小管断裂行显微接管修复手术成功的关键是寻找下泪小管断端，硅胶条是较为理想的支撑物，有利于管壁愈合且能避免管腔狭窄。

Efficacy analysis of anastomotic microsurgery of the lacrimal canaliculus in 43patients with traumatic lower lacrimal canalicular rupture

Objective To analyze the therapeutic efficacy of anastomotic microsurgery of the lacrimal canaliculus in 43 patients with acute or old traumatic laceration of lower lacrimal canaliculus. Methods Through surgical microscope, the broken end of lower lacrimal canaliculus was found near the wound closed to the nose. A silastical bar was inserted into the lower lacrimal canaliculus from the broken end to the nasal cavity. The lacerated lower lacrimal canaliculus was then repaired. Results Among the 43 patients, 4 were reoperated by other method because their silastical bars were dislocated. All patients recovered,the successful rate achieved 90.7%. Conclusion Acute patients should be operated early, searching for the broken end of the lower lacrimal canaliculus is the key point. Silastical bar is an ideal support which can accelerate healing of the canalicular wall and avoid stenosis of the cavity.
