胃转流术在胃部疾病合并2型糖尿病的应用

目的探讨胃转流术对胃部疾病合并2型糖尿病术后的血糖及并发症的影响。方法选取42例胃部疾病（包含良性病及恶性病）合并2型糖尿病患者行胃部切除术或胃全切，消化道重建采用Roux-en-Y胃转流术式。对其临床资料进行回顾性分析。结果42例手术均获成功。术后1月与术前空腹血糖比较，差异有统计学意义（P〈0．05）。术后3月与术后1月的早晨空腹血糖比较，差异无统计学意义。术后并发症出现胃瘫综合征9例，切口液化5例，未出现吻合口瘘及术后出血。结论胃转流术是治疗胃部病变合并2型糖尿病的一种有效的治疗方法。术后胃瘫综合征是胃转流术治疗2型糖尿病术后最常见并发症，其次为切口脂肪液化，以上情况术前采取预防措施、术后采用积极治疗手段均可治愈。

Application of gastric bypass on gastric diseases complicated with type 2 diabetes

Objective To investigate the influence of gastric bypass on gastric diseases complicated with type 2 diabetes. Methods A total of 42 cases of gastric disease were selected. Patients complicated with type 2 diabetes were performed subtotal gastrectomy or cardinal gas- trectomy, alimentary tract reconstruction by Roux-en-Y＇s operation and their clinical data were retrospectively analyzed. Results All operations were successful. There were significant differ- ences between 1 month and preoperative fasting blood glucose（P〈0.05）. The effect at 1 month was similar to that at 3 months. Conclusion Roux-en-Y gastric bypass is an effective method of treatment on gastric diseases complicated with type 2 diabetes. Postoperative gastroparesis syn- drome is the most common postoperative complications, followed by incision fat liquefaction. If we take preventive measures to above situation preoperatively, and adopt effective therapy meth- ods, all of them can be cured.
