几种严重创伤早期并发症的防治

目的：总结严重创伤早期并发症的诊治经验。方法：回顾1985年5月－2000年12月收治847例严重创伤中282例主要并发症病例资料。其中急性呼吸窘迫综合征（ARDS）115例，多器官功能不全综合征（MODS）89例，应激性溃疡伴出血78例，对诊断治疗方法及疗效进行了分析。结果：ARDS治愈率68．7％（79／115），MODS治愈率57．3％（51／89），应激性溃疡治愈率67．9％（53／78），结论：MODS的治疗主要以脏器支持为主，减轻早期应激反应，积极抗休克及感染，ARDS的治疗应及时正确采用呼吸机通气，尽快纠正低氧血症，积极处理原发病，应激性溃疡以非手术治疗为主，对严重大出血非手术治疗无效者应积极手段治疗。

Prevention and treatment of several kinds of early complications of serious wound

Objective To review the experience of diagnosis and treatment of early complications of serious trauma.?Methods Two hundreds and eighty two patients with complications of serious wound from May 1985 to Dec 2000 were analysed.Among them,there were 115 cases of ARDS,89 cases of MODS,and 78 cases of stress ulcer with hemorrhage.?Results Cure rate of ARDS , MODS , and stress ulcer were 68.7%(79/115), 57.3%(51/89), and 67.9%(53/78)respectively.?Conclusion MODS should be treated mainly by organ support,relieving early stress, and anti hock. ARDS should be treated by prompt mechanical ventilation,redressing hypoxemia,and treatment of primary diseases. Stress ulcer should be treated mainly by medication.Operation is recommended when medication is inefficient.
