高龄初产妇并发重度子痫前期对母儿影响的临床分析

目的:探讨高龄初产妇并发重度子痫前期对母儿预后的影响。方法:以该院2001年4月～2008年4月收治的重度子痫前期单胎妊娠年龄≥35岁的初产孕妇26例为观察组,同期重度子痫前期单胎妊娠年龄<35岁的初产孕妇107例为对照组,对两组患者资料进行回顾性分析,比较两组孕妇及围生儿的最终结局。结果:观察组内科合并症发生率较对照组高,差异有统计学意义(P<0.05),产后出血、胎盘早剥发生率两组比较差异无统计学意义(P>0.05)。观察组低体重儿出生率、新生儿窒息和围产儿死亡率较对照组高,差异有统计学意义(P<0.05)。结论:高龄初产妇并发重度子痫前期对母儿危害大,应加强产前监护,适时终止妊娠,以改善母儿预后。

The effects of elderly primipara complicated by severe preeclampsia on the mother and infant:clinical analysis

Objective:To study the effects of elderly primipara complicated by severe preeclampsia on the maternal and neonatal prognosis.Methods:The clinical datas of 26 cases of single pregnancy and elderly primiparas(age≥35 years)delivered in this hospital from April 2001 to April 2008 were analysed retrospectively as the observed group.The younger primiparas complicated by severe preeclampsia delivered at the same time were used as the control group.Results:The rates of internal complication in the observed group were significantly higher than that of the control group(P<0.05).No significant differences were shown in the placental abruption and postpartum hemorrhage between the two groups(P>0.05).The rates of the low birth weight,neonatal asphyxia rate and perinatal mortality in the observed group were significantly higher than that of the control group(P<0.05).The maternal and fetal outcomes of the two groups were compared.Conclusion:Elderly primipara complicated by severe preeclampsia is harmful for the mother and infant.The prematal custodial care should be strengthened and pregnancy should be stopped properly.
