彩色多普勒超声在肝脏肿瘤鉴别诊断中的应用

目的： 探讨彩色多普勒超声 （ＣＤＦＩ） 在原发性肝癌 （ＨＣＣ）、转移性肝癌 （ＨＬＣ） 及肝血管瘤（ＨＨＡ） 鉴别诊断中的临床应用价值。方法： 通过１００ 例肝肿瘤患者及４０ 例正常对照组 （ＮＳ） ＣＤＦＩ参数对比分析，比较ＨＣＣ、Ｍ ＬＣ 和ＨＨＡ 各组间ＣＤＦＩ影像学差异及特点。结果： （１） ＨＣＣ 组与其他各组比较肿瘤内部及周边部动脉血供明显丰富 （Ｐ＜ ０．００５）， 肝动脉直径明显增粗， 收缩期最大流速和舒张期最小流速明显增快 （Ｐ＜ ０．０１）； （２）Ｍ ＬＣ 组与ＨＨＡ 组比较肿瘤周边动脉血流检出率显著增高（Ｐ＜ ０．００５）， 内部血供无明显差异， 肝动脉直径及血流速度与ＮＳ组比较三组之间无明显差异 （Ｐ＞ ０．０５）； （３） 阻力指数 （ＲＩ） 四组之间无显著差异 （Ｐ＞ ０．０５）。结论： ＣＤＦＩ在肝脏肿瘤鉴别诊断中有重要的临床应用价值。

Evaluation of Color Doppler Flow Imaging in Differential Diagnosis of Liver Tumors

Objective:Evaluation of color Doppler flow imaging(CDFI)in the differentiation of hepatic cellular carcinoma(HCC),metastatic liver cancer(MLC)and hepatic hemangioma(HHA).Methods:We used CDFI to examine 60 HCCs,20 MLCs,20 HHAs and 40 controls to compare the differentiation indices in this 4 groups.Results:(1)HCC demonstrated richer intratumoral and peritumoral blood flow( P <0 005).The hepatic artery diameter was significantly widened.The systolic and diastolic velocity were obviously higher compared with other groups ( P <0 01,respectively).(2)MLC showed a higher blood flow around than HHA.No significant difference in diameter and velocity of the hepatic artery between MLC,HHA and control was found( P >0 05).(3)Resistant index(RI)had no significant difference between the 4 groups( P >0 05).Conclusion:CDFI plays a key role in differentiation of HCC,MLC and HHA.
