超声冲浪式浸浴治疗烧伤残余创面的疗效观察

目的探讨超声冲浪式浸浴治疗烧伤残余创面的护理措施和方法，寻找烧伤后期最有效的康复手段。方法我科从2009年7月～2012年5月使用河南省南阳国防科技工业电气研究所研制生产的华灯牌GSX—SLl型移动式浸浴水疗槽，对18例烧伤残余创面患者（实验组）进行超声冲浪式浸浴治疗。对照组18例采用传统手工清创换药进行治疗。结果实验组18例患者烧伤残余创面经超声冲浪式浸浴治疗后，创面新鲜清洁，上皮生长速度明显加快。16例患者在较短时间内创面自行愈合，且愈后皮肤平整有弹性，疤痕增生不明显，2例患者经浸浴治疗3～4次后，创面感染控制．肉芽新鲜经积极手术植皮治疗后完全愈合。对照组18例患者经传统手工清创换药数次后，11例患者自然愈合，7例经手术植皮后愈合．对照组换药愈合时间较长，愈后皮肤疤痕明显增生。结论超声冲浪式浸浴治疗能有效清洁烧伤残余创面，减少创面的细菌和毒素，改善创面和创周的血液循环，促进创面愈合，缓解疤痕增生，同时减轻换药或功能锻炼时给患者带来的痛苦。

The effect of ultrasound surf-bath treatment on residual burn wound

Objective To explore methods and measures on curing residual burn wound by ultrasound surf-bath treatment and find the most effective post rehabilitation ways of bum wound. Methods Ultrasound surf-bath treatment was proceeded on 18 patients with residual burn wound in our department during Jul 2009 and May 2012,using GSX-SL1 mobile bath spa tank made in Henan Nanyang Electric Institute of National Defense Industry. Simultaneously, two groups of patients, EG （experiment group） and CG （control group）, 18 members for each group, were selected randomly to take different treatment, ultrasound surf-bath to EG,traditional manual debridement and dressing treatment to CG. Results After having taken ultrasound surf-bath treatment,the residual burn wound of the 18 EG patients was fresh and clean, epithelial growth was significantly faster; and the wound of 16 patients healed spontaneously in a relatively short period of time with smooth elastic and unobvious scar hyperplasia skin. The wound infection of two patients was controlled after 3 or 4 times treatments and the granulation was fresh,the wound was completely healed after active skin grafting. For the 18 CG patients, after having taken traditional manual debridement and dressing treatment several times,the wound of 11 persons healed spontaneously, the wound of seven persons healed after skin grafting. The healing time for the wound of CG patients was longer and scar hyperplasia was obvious after healing. Conclusion By ultrasound surf-bath treatment, the residual burn wounds can be cleaned effectively, bacteria and toxins can be reduced, blood circulation of the wound and around the wound can be improved, the healing of the wound can be promoted, scar hyperplasia can be relieved and meanwhile the patients suffer much less from dressing or functional
