改良舌骨下颌骨悬吊术在阻塞性睡眠呼吸暂停手术中的应用

目的 探讨改良舌骨下颌骨悬吊手术在治疗睡眠呼吸暂停低通气综合征（OSAHS）的手术方法和效果。方法 68例OSAHS患者采用同期或者分期行改良舌骨下颌骨悬吊术，术后6个月复查多导睡眠监测(PSG)和上气道螺旋CT。结果 68例术后睡眠监测指标均较术前有明显改变, 参照2009年阻塞性睡眠呼吸暂停低通气综合征诊断和外科治疗指南行疗效判断, 术后6个月的有效率为100%；术前和术后螺旋CT检查发现舌后区前后径减小，左右径增大,更符合正常人气道形状。结论 改良舌骨悬吊术是治疗舌咽平面狭窄的OSAHS患者的有效方法。

The application of modified hyoid suspension surgery for the obstructive sleep apnea-hypopnea syndrome

Objective To discuss the method and therapeutic effects of modified hyoid suspension surgery for obstructive sleep apnea-hypopnea syndrome. Methods Modified hyoid suspension was performed in 68 OSAHS patients in one or consecutive stages. All the patients underwent polysomnography(PSG) and multislice CT (MSCT) before and six months after the operation. Results In all the cases, PSG indexes showed significant improvement after the operation. According to the assessment guidelines, the efficiency is 100% six months after. The anteroposterior dimensions of retroglossal were decreased and transverse dimensions were increased after the surgery. The shape of airway tended to be normal. Conclusion Modified hyoid suspension is effective for OSAHS patients with oropharyngeal obstruction.
