慢性萎缩性胃炎辩证分型与胃黏膜改变的相关性

目的探讨慢性萎缩性胃炎（chronic atrophic gastritis,CAG）微观辩证的可行性。方法回顾性收集70例CAG患者临床资料,分析CAG中医证型与其胃黏膜相、病理组织学表现之间的关系。结果 CAG患者：①证属胃阴不足型,其黏膜萎缩程度多属重度;肠上皮化生现象较为普遍。②脾胃虚弱型较为常见,但其黏膜的萎缩程度多较轻。③内镜下见胆汁反流者,临床证型多为肝胃不和型。④内镜下见糜烂者,大多证属脾胃湿热型。结论 CAG的证型与胃黏膜镜下表现和病理组织学改变之间存在一定的联系和规律性。

Relation between Differentiation and Typing of Chronic Atrophic Gastritis and Mucosa Changes

Objective To discuss the feasibility of microcosmic differentiation and typing of chronic atrophic gastritis （CAG）. Methods Materials of 70 CAG patients were collected and retrospectively analysed. Patients were divided into five groups. The relation between their differentiation and typing and histopathological manifestations were analysed. Results ① If the CAG patients were with stomach-yin deficiency,their shrink level of stomach membrane was usually serious and intestinal metaplasia was common. ② Deficiency of spleen and stomach was very normal but the shrink level of stomach membrane was light. ③ If there existed bile reflux under endoscope,the patients were usually with liver-stomach disharmony. ④ If there existed erosion under endoscope,most patients were with dampness-heat in spleen and stomach. Conclusion Differentiation and typing of CAG are related with endoscope observation of stomach membrane and histopathological changes.
