替比夫定治疗慢性乙型病毒性肝炎患者定量检测HBeAg的意义

目的探讨替比夫定治疗的慢性乙型病毒性肝炎(chronic viral hepatitis B,CHB)患者血清HBeAg水平对抗病毒疗效的预测作用。方法免疫化学发光法和荧光定量聚合酶链反应技术分别检测78例HBeAg阳性患者在替比夫定治疗过程中血清HBeAg、HBV DNA水平的变化,统计学分析基线及治疗期间HBeAg水平与HBV DNA转阴的关系。结果治疗48周,HBV DNA转阴66例(84.6%),HBeAg转阴28例(36.6%);基线HBeAg水平<436 s/co或治疗12周时HBeAg下降>2.5 log10的患者更易发生HBV DNA转阴及HBeAg转阴。结论在替比夫定抗病毒治疗过程中检测HBeAg水平可用于评估抗病毒治疗效果。

The Quantitative Assay of HBeAg in Patients with HBeAg-positive CHB Receiving Telbivudine Therapy

Objective To explore the role of serum HBeAg quantification in predicting antiviml response in CHB patients treated by telbivudine. Methods Seventy-eight patients with HBeAg-positive CHB were included in the present study. Emzyme chemiluminescence immunoassay and real-time fluorescent quantitative PCR were used to assay the level of serum HBeAg and HBV DNA load during telbivudine treatment, respectively. The relationship between the amount of HBeAg at the baseline and during telbivudine therapy and the disappearance of HBV DNA or HBeAg was analyzed by using statistical software. Results After 48 week of telbivudine therapy, HBV DNA turned negative in 66 CHB patients （84.6%） and HBeAg was not detected in 28 CHB patients （36.6%）. HBV DNA easily turned negative and HBeAg was easier disappeared in patients whose HBeAg level was under 436 s/co at the baseline or HBeAg level decreased 〉2.5 logl0 at week 12 of treatment. Conclusion Quantitative assay of HBeAg in patients with HBeAg-positive CHB receiving telbivudine therapy might be used to evaluate the antiviral efficacy.
