螺旋CT三期扫描对小肝癌的诊断

目的 研究小肝癌(SHCC)在螺旋CT三期(动脉期、门脉期和延迟期)增强扫描的表现，比较各期的检出率，提高诊断水平。方法 12例患者以2．5～3．0ml／s注射100ml对比剂，分别于注射后25s、70s及5min后分别行动脉期、门脉期及延迟期扫描，观察SHCC在各期的强化方式，统计各期的检出率。结果 共发现15个病灶，动脉期，门脉期和延迟期的检出率分别为80％、60％和73％。动脉期敏感性高于门脉期和延迟期，三期联合检出率明显增加达到93％。结论 SHCC的增强表现形式多样，螺旋CT增强扫描可充分显示SHCC各期的增强表现，正确评价其血流变化，提高病灶的检出率及诊断的准确性。

Multiphase spiral CT detecting small hapatocellular carcinoma

Objective To investigate the features of small hepatocellular carcinoma (SHCC) on enhanced multiphase spiral CT scanning and to analyze its sensitivity in different phases. Methods 12 patients underwent multiphase contrast enhanced scanning after intravenous administration of contrast medium(100 ml)at a rate of 2.5-3 ml/s.The scan started for the arterial phase,portal venous phase and delayed phase at 25 s,70 s and 5 min respectively. The enhancement pattern of each lesion in three phases was analyzed and the sensitivity was compared. Result Of the 15 SHCCs detected, the sensitivity in three phases was 80 %, 60 % and 73 % respectively. The combined sensitivity increased significantly to 93 % by combining the detection in three phases. Conclusion Multiphase contrast enhanced spiral CT can demonstrate the patterns of SHCC and improve its detection rate.
