| 摘 要： | 目的：探讨保护和加强脊髓栓系综合征(TCS)患儿的脊柱结构及稳定性的手术治疗方法。方法与结果：共收治TCS患儿197例。92％的TCS患儿(182／197)合并有脊柱裂。54例采用非椎板双开门治疗，随访发现1例脊柱后突加重，1例外伤后脊髓受损，4例(7．4％)术后发生脑脊液漏，143例采用椎板双开门暴露椎管术后缝合，或脊柱裂术后椎板植骨等改进手术方法，损伤小，手术暴露好，术后脊柱结构完全或部分修复，治疗有效，并发症少(术后2例(1．4％)发生脑脊液漏)。结论：椎板双开门打开椎管探查脊髓行松解术及术后植骨闭合脊柱裂等方法暴露清楚，松解彻底，既可保护和加强脊柱结构，又可减少并发症，是手术治疗TCS值得推荐的方法。

| Abstract: | Aim: To improve the treatment effective of tethered cord syndrome ( TCS) in children by protecting and strengthening the structure and stability of spine. Methods and Results: Among 197 cases of TCS, 92% (182/197) was associated with vertebral column split. A total of 143 cases were treated by opening double doors or planting bone in laminae, and 54 were treated by traditional method. The new method was better in exposure with milder injury and less complications. Conclusion : The operation of opening double doors or planting bone in laminae is a better way in treating TCS. |
