论BIPAP呼吸机治疗高龄COPD合并PCP患者的护理研究

目的探讨BIPAP呼吸机治疗高龄慢性阻塞性肺炎（COPD）合并卡氏肺囊虫肺炎PCP患者的护理方法及其效果。方法回顾性分析了2007年叭月至2011年01月入住我院的4例高龄COPD合并PCP患者的临床资料，对其实施常规吸氧、解痉平喘等处理，应用激素与抗感染等治疗手段，同时给予其鼻（面）罩BIPAP机械通气治疗。主要对治疗前后患者尿液中的IgA、IgG、IgM、动脉血气变化、心率、呼吸频率及血压等进行比较分析，数据用卡方检验（检验）的方法进行检验，组间差异以P〈0．05表示具有统计学意义。结果（1）本组患者在治疗前尿液中的IgA、IgG、IgM含量分别为（1．4±0．5）mg／L、（0．8±0．3）mg／L、（0．1±0．02）mg／L，治疗后上述三个指标值分别为（0．7±0．3）mg／L、（0．3±0．2）mg／L、（0．02±0．01）mg／L，前后存在显著的统计学差异（P〈0．01）；（2）治疗前患者PO2、PCO2及SO2（％）分别为（7．56±1．62）kPa、（9．69±1．52）kPa、（83．2±5．2），治疗后上述各值分别为（9．38±1．57）kPa、（8．13±1．29）kPa、（95．7±3．28），上述各个指标在治疗前后存在统计学差异（P〈0．05）；（3）治疗前患者呼吸频率（R，次／分）、心率（次／分）、SBP（次／分）、DBP（次／分）分别为（29．3±5．6）、（119．3±13．2）、（143．9±11．2）、（79，3±7．4），治疗后患者上述指标值分别为（17．1±3．2）、（83．3±8．2）、（139．5±10．8）、（78．9±7．2），上述各指标中，治疗前后存在统计学差异的为呼吸频率、心率（P〈0．05），后两者在治疗前后不存在统计学差异（P〉0．05）。结论运用BIPAP呼吸机治疗及护理观察高龄COPD合并PCP患者，疗效显著且安全，应该在临床上加以推广并应用。

Study on nursing experience for the elderly patients with COPD supervened with PCP treated with BIPAP ventilator

Objective To explore the nursing methods and clinical effects of BIPAP ventilator on the elderly patients with COPD supervened with PCP. Methods The clinical data of 4 patients with COPD supervened with PCP were analyzed retrospectively in our hospital from January 2007 to January 2011. They were treated with routine oxygen, spasmolysis and antiasthmatic, hormone and anti -itffective methods, at the same time received the treatment with nasal （facial） mask BIPAP ventilator. Comparative analysis was done before and after treatment in the urine of patients with IGA, the IGG, IgM, arterial blood gas, heart rate, respiratory rate and blood pressure data using the chi - square test. Results There were statistic differences in IGA, ICG and ICM of the urine, PO2, PCO2, SO2, respiratory rate and heart rate before and after treatment （ P 〈 0. 01 ）. Conclusions The BIPAP ventilator should be used to treat and care the elderly patients with COPD supervened with PCP. It has significant effects and safety. It is worthy spreading in clinical practice.
