乙型肝炎病毒大蛋白在血清HBeAg阴性HBV感染者中的检测意义

目的探讨乙型肝炎病毒大蛋白（HBV-LP）在血清HBeAg阴性HBV感染者中的检测意义。方法对162例HBV感染者血清采用酶联免疫吸附试验检测乙型肝炎病毒大蛋白及乙型肝炎病毒标志物,FQ-PCR定量检测HBV-DNA。结果162例HBV感染者血清中,HBV-LP与HBV-DNA、HBeAg之间均关联显著（P均〈0.01）;HBV-LP与HBV-DNA、HBeAg间阳性率均存在显著性差异（P均〈0.05）;HBV-LP阳性率为84.57%,较HBV-DNA、HBeAg高,分别为62.96%、38.89%（P均〈0.05）;血清HBV-LP浓度及其阳性率在HBeAg和HBV-DNA的阳性组与阴性组间均存在显著性差异（P均〈0.05）;HBeAg阳性组中HBV-LP与HBV-DNA间阳性率无显著性差异（P〉0.05）,检测符合率为96.83%;HBeAg阴性组中HBV-LP与HBV-DNA间阳性率存在显著性差异（P〈0.05）;HBV-LP阳性率为76.77%,较HBV-DNA高。在58例HBeAg和HBV-DNA共同阴性的HBV感染血清中,检出HBV-LP阳性42例。结论血清HBV-LP是监测血清HBeAg阴性HBV感染者体内病毒复制、疾病进程与疗效的敏感指标。

Clinical Significance of Detecting Serum Hepatitis B Virus Large Surface Protein in Patients with HBeAg-negative Hepatitis

Objective To discuss the clinical significance of serum Hepatitis B Virus Large Surface Protein（HBV-LP）detection in patients with HBeAg-negative hepatitis.Methods Serum Hepatitis B Virus Large Surface Protein was measured by ELISA as HBV M and HBV-DNA by fluorescence quantitative-PCR technique in 162 patients with Hepatitis B.Results In 162 serum samples of the patient with HBV infected,There were high close relations hips among serum HBV-LP,HBV-DNA,HBeAg（P〈0.01）.HBV-LP had significant difference with HBV-DNA,HBeAg respectively（P〈0.05）.HBV-LP positive rate was 84.57% and it was higher than that of HBV-DNA,HBeAg respectively（62.96%,38.89%）.The content and positive rate of serum HBV-LP was significant difference between HBeAg-positive and HBeAg-negative group,and also in HBV-DNA.The positive rate of HBV-LP and HBV-DNA was no difference in HBeAg-positive group（P〉0.05）,their match rate was 96.83%,and significant difference in HBeAg-negative group（P〈0.05）.HBV-LP positive rate was 76.77%,higher than that of HBV-DNA.There were 42 positives of HBV-LP in 58 negative serums of HBeAg and HBV-DNA.Conclusion HBV-LP is more sensitive as a serum marker of monitoring HBV replication and the disease process and the effectiveness of treatment in HBeAg-negative group with HBV inflected.
