主动脉球囊反搏治疗经皮冠状动脉介入术后心功能低下

目的 观察主动脉球囊反搏（IABP）在急性心肌梗死（AMI）经皮冠状动脉介入治疗（PCI）术后心功能低下的疗效。方法 选择急性心肌梗死PCI术后合并心力衰竭病人20例，所有病人均静脉使用血管活性药物。当药物已不能维持有效血循环时由股动脉置入主动脉球囊反搏导管，行主动脉球囊反搏术。观察尿量、心率、意识改变，放置漂浮导管测连续心排血量（09）、有创动脉收缩压（SBP）、有创平均动脉压（MABP）、心脏指数（CI）及肺毛细血管楔压（PCWP）。结果 主动脉球囊反搏术后，SBP、MABP、CO、CI较术前明显升高（P〈0．05）。16例心功能不同程度改善，尿量增多，心率减慢，2例改善不明显，2例病亡。结论 主动脉球囊反搏术能有效地改善PCI术后低心排血量病人的心功能。

The Efficacy of Intro-aortic Balloon Pump for Heart Failure after Percutanous Coronary Intervention

Objective To evaluate the effect of intro-aorbic ballon pump (IABP) for acute myocardial infarction (AMI) complicated with heart failure after percutanous coronary intervention (PCI).Methods Twenty patients with AMI were complicated with heart failure after PCI.When effective circulation could not be maintained by vasoactive agents, the catheter was put by femoral approach, Volume of urine and heart rate, consciousness were monitored.Continuous cardiac output(CO),systolic blood pressure mean arterial pressure(MABP),cardial index(CI),pulmonary capillary wedged pressure(PCWP) were measured.Results SBP,MABP,CO,CI were remarkaby higher after IABP.PCWP was remarkably lower after IABP.Heart rate slower and volume of urine increased.Conclusion IABP could improve cardiac function after PCI effectively.
