| 摘 要： | 目的总结采用下咽胃吻合治疗颈及高位胸上段食管癌的经验及效果。方法回顾分析18例应用胃吻合重建消化道治疗食管癌的临床资料。均采用食管全切除，胃经食管床上徙至颈部行下咽胃吻合。结果全组无手术死亡，术后发生颈部吻合口瘘并腹部切口裂开中1例，喉返神经损伤3例，颈部切口感染1例。随访6月-6年，平均2年，3例分别于术后3～5个月发生吻合口不同程度的狭窄，均行扩张后治愈；切端癌残留1例。结论下咽胃吻合治疗食管癌成功的关键是直视下游离、良好的吻合技术和胃组织及血供的充分保护，其手术在基层医院可行并行益。
