摘    要:目的 探讨MRI对出血坏死性鼻息肉的诊断价值。方法 观察14例经手术病理证实的出血坏死性鼻息肉临床资料及MRI表现。结果 14例出血坏死性鼻息肉均源自单侧上颌窦,以上颌窦口为中心向上颌窦-中鼻道生长,MRI信号混杂,13例T2WI显示病变边缘见明显不规则低信号环;3例增强扫描表现为"菜花"状不均匀显著强化,T2WI低信号环无强化。所有病变均有不同程度的压迫性骨质吸收,上颌窦周围软组织未见受侵征象。结论 出血坏死性鼻息肉的MRI表现具有一定的特征性,可作为该病首选检查方法,对指导临床治疗有重要价值。

Abstract:Objective To explore the value of MRI in diagnosis of hemorrhagic and necrotic nasal polyps. Methods Fourteen patients with hemorrhagic and necrotic nasal polyps were confirmed by pathology after surgery. The clinical data and MRI findings were retrospectively analyzed. Results The lesions were all derived from unilateral maxillary sinus. The maxillary sinus ostia was being the center. The lesions extended into maxillary sinus and middle nasal meatus. MR images showed heterogeneous signals. The irregular hypointense signal rims were found on the periphery of 13 lesions on T2WI. Contrast images showed significant heterogeneous enhancement like cauliflower pattern in 3 lesions, and there was no enhancement with the irregular hypointense signal rims on T2WI. All the lesions displayed compression bone absorption with various degrees, and did not invade the peripheral soft tissue. Conclusion Hemorrhagic and necrotic nasal polyps shows characteristic findings with MRI, which should be the first image choice for its diagnosis and can take great advantages of the clinical treatment.
