腕头状骨移位治疗晚期月骨无菌性坏死的应用解剖与临床研究

目的:推荐一种新的治疗晚期月骨无菌性坏死的手术方法。方法:对50侧新鲜上肢标本解剖观察腕部的血管构造、对100例腕头状骨和月骨进行对比观测,以此设计带血管蒂的头状骨替代月骨术,临床应用16例,术后随访1～12年。结果:典型相关分析证明头状骨与月骨的几何形态、外径和关节面的弧长、弧高极其相似,相关性显著,可移位替代。临床长期随访证明本术式即切除了坏死碎裂的月骨、消除了腕痛,又保持了腕骨的稳定和腕关节的功能。结沦:本术式对治疗晚期月骨无菌性坏死是一种疗效好、副作用小、功能满意的手术方法。

APPLIED ANATOMICAL AND CLINICAL STUDY ON CARPAL CAPITAT TRANSFERRING IN TREATING ADVANCED ASEPTIC NECROSIS OF LUNATE BONE

Olijective: To design a new method for treament advanced aseptic necrosis of lunate. Methods:The construction of the carpal vessels was observed on SO sides of autopsies. the comparative measure of capitate and lunate was carried out on 100sides of autopsies. Designed a operation of capitate with pedicle transferring in substitute of lunate and applied in 16 cases clinically with follow- up of 1 ~ 12 years. Results:The external diameter and the arch height and length of the articular surface of capitatum and lunate bone were similar. Capitate can be transferred in substitute of the necrostic lunate. Follow- up verified that the operation not only cut out the decayed lunate bone and alleviated the pain but also preserved the function and equilibration of the carpal joint. Conclusions: this method was effective treating advanced aseptic necrosis of lunate bone.
