摘    要:目的探讨不同术式剖宫产术后对再次手术的影响,论证新式剖宫产术不缝合腹膜和膀胱反折腹膜的可行性。方法对109例不同术式剖宫产患者再次手术中情况进行回顾性分析,对盆腔粘连程度、总手术时间、开腹至胎头娩出时间、及术中取头困难产钳助娩率、术后并发症进行比较。结果传统子宫下段剖宫产与新式剖宫产术,两组总手术时间、开腹至取头时间、盆腔粘连程度、术中产钳助娩率、术后并发假性结肠梗阻差异有统计学意义(P<0.01)。而产后出血差异无统计学意义(P>0.05)。结论新式剖宫产术后再次手术盆腔粘连严重,对再次手术影响较大,应进行改良,视患者情况选择合适手术方式。

Abstract:Objective To explore the effects of different methods of cesarean section on the reoperation,and study the feasibility of stark's cesarean section without suture of peritoneum and cesarean section with junction procedure of anterior. Methods To analysis the condition of 109 cases of women in labour in reoperation,comparing clinical observation of stark cesarean section,the amount of operating time,the time of fetuing out,difficulty of head delivery and postoperative complications with eath other. Results The research in clinical observation of stark cesarean section,the amount of operating time,the time of fetuing out,the rate of forceps delivery,acute colonic pseudoobstruction after surgery were significant(P < 0.01).On the other hand, the comparison between postpartum hemorrhage were insignificant(P > 0.05). Conclusion The severity of clinical analysis of post operation pelvic adhesion plays an important role in reoperation,therefore,doctors should improve the methods and select the best way accordding to the condition of patients.
