摘    要:目的:通过对观察Pitt评分与APACHEⅡ评分系统对呼吸机相关性肺炎的发生及预后的回顾性调查。方法:不同的评分系统观察2008年6月-2011年6月在我院住院并予以有创呼吸机辅助治疗病患VAP的发生率及死亡率进行回顾性整理调查及统计分析,并运用统计学方法比较两套系统的优劣,找到适宜临床的快速评估方法,以及对病患预后进行早期评估,并对其敏感性进行比较。结果:对94例患者分别使用Pitt评分系统和APACHEⅡ评分进行上机前得分评估,发现两者同样具有较高的准确性,两者具有相关性,相对于多项实验室结果Pitt评分具有更加快速、简便、易掌握、准确的特点。结论:比较Pitt评分与APACHEⅡ评分系统发现,前者具有简洁,容易掌握的优点,且无需临床辅助资料即可得出较高的针对危重病患VAP发生率及死亡率的预测效果,较之APACHEⅡ评分系统有更加简便、实用的临床使用价值。

Abstract:Objective:This study compares the effectiveness of the Pitt bacteremia score and the Acute Physiology and Chronic Health EvaluationⅡ(APACHE Ⅱ)scoring systems for the prediction of mortality and the ratio of VAP in critical patients.Methods:A retrospective study was conducted in patients who had received mechanical ventilation more than 48 hours from June 2008 to June 2011.Compared the effectiveness of Pitt bacteremia score and APACHE Ⅱscoring system in statistics.Results:In 94 patients respectively,using the Pitt score system and the APACHE Ⅱscore for the former scoring assessment,found both with higher accuracy,both have relativity,relative to the number of laboratory results of Pitt score has a more rapid,simple,easy master,accurate characteristic.Conclusion:Pitt bacteremia score is significantly correlated with the APACHE Ⅱscoring system.The Pitt bacteremia score easier to control.Pitt bacteremia score is significantly correlated with the APACHE Ⅱscoring system.
