青光眼激光术后解剖结构改变的研究

摘    要:目的 观察原发性闭角型青光眼激光术后A超显示的解剖结构改变.方法 90例(90眼)原发性闭角型青光眼(PACG)纳入本研究,随机被分为两组,B组采用360.虹膜成形联合YAG激光周边虹膜切除术;A组采用YAG激光周边虹膜切除术.应用A型超声测量PACG激光前后前房深度、晶状体厚度、眼轴长度;并做眼压测量.各项均值进行组间比较和分析.结果 在A组中,除晶状体厚度外,各项均值(前房深度、眼轴长度、眼压)与术前相比,差异有统计学意义(t=2.438,P<0.01;t=-2.135,P<0.05;t=2.524,P<0.01).在B组中,除前房深度和晶体厚度外,术后,眼轴、眼压比术前降低(t =2.517,P<0.01;t=2.679,P<0.01).眼轴缩短的幅度与眼压的降低的幅度成正相关(r =0.713,P<0.01).结论 原发性闭角型青光眼激光术后眼轴缩短,眼压降低,眼轴的缩短与眼压的降低有关.

Abstract:Objective To observe change of the anatomical structure of primary angle-closure glau- coma (PACG) after laser surgery by A-mode ultrasonic scanning. Methods Anatomical structures inclu- ding the anterior chamber depth(ACD) , lens thickness (LT) and the axial length (AL) , were measured by A-mode ultrasonic scanning in 90 eyes of 90 patients with PACG before and after laser surgery. 90 patients were randomly divided into two groups, Group A underwent Nd : YAG laser iridectomy, while group B did the combined surgery of laser iridoplasty around 360°, followed by Nd : YAG laser iridectomy. All mean values were analyzed with t-test. Results In group A, the eyes had significant differences in all items except LT compared with that before surgery (t = 2. 438, P 〈 0. 01 ; t = - 2. 135, P 〈 0. 05 ; t = 2. 524, P 〈 0.01 ). While in group B, there was a statistically significant difference in IOP and AL after surgery (t =2. 517, P 〈0. 01 ; t = 2. 679, P 〈0.01). The reduction of AL had positive relationship with IOP reduction (r = 0. 713 ,P 〈 0.01 ). Conclusion There is a small but statistically significant decrease in AL and IOP after laser surgery. This reduction of AL was related to IOP reduction.
