对比观察高位硬膜外麻醉与气管内全麻在乳腺癌根治术中的应用

目的:探讨高位硬膜外麻醉与气管内全麻对乳腺癌根治术的麻醉效果及不良反应。方法:选择66例乳腺癌根治术病人,随机分为观察组和对照组,观察组采用硬膜外腔阻滞,对照组采用气管内全麻。观察两组各个时间段体内激素变化情况。结果:观察组的血浆去甲肾上腺素、内皮素和血清皮质醇术中、术后比较无统计学意义(P>0.05),与对照组比较有统计学意义(P<0.01)。两组的血浆肾上腺素术中、术后均无统计学意义(P>0.05)。两组的血清C-反应蛋白术中、术后均明显升高(P<0.01),两组比较无统计学意义(P>0.05)。结论:高位硬膜外腔阻滞麻醉乳腺癌根治术效果满意,优于气管内全麻,不良反应少,值得借鉴。

Comparative Observation of High Epidural Anesthesia and Endotracheal General Anesthesia in Radical Surgery of Breast Cancer Application

Objective:To investigate the high thoracic epidural anesthesia and endotracheal general anesthesia for radical mastectomy for breast cancer the anesthesia effect and adverse reaction.Methods:66 cases of breast cancer patients undergoing radical surgery,were randomly divided into the observation group and the control group,the observation group was treated with epidural block,the control group using endotracheal anesthesia.Observation of the two groups of the periods of time in vivo hormone change.Results:The observed group plasma noradrenaline,endothelin-1 and serum cortisol during and after the operation,there is no significant difference(P>0.05),compared with the control group had statistical significance(P<0.01).Two groups of plasma epinephrine during and after the operation,there were no statistical significance(P>0.05).Two groups of serum C-reactive protein for postoperative,were significantly higher(P< 0.01),comparison of the two groups was not statistically significant(P>0.05).Conclusion:High epidural block anesthesia in radical operation of breast cancer with satisfactory results,better than endotracheal general anesthesia,fewer adverse reactions,worth learning.
