养阴清热 补肾化瘀中药联合CTX治疗狼疮性肾炎的临床研究

目的:观察养阴清热,补肾化瘀法联合环磷酰胺(CTX)治疗狼疮性肾炎的疗效。方法:采用随机、平行、对照比较方法,分中医组、中西医组两组,均为上海市中医医院风湿科门诊和住院病人。每组各30例,共60例,疗程6个月,对蛋白尿及免疫指标进行观察。结果:(1)总有效率分别为中医组96.7%,中西医组93.33%。(2)两组在治疗蛋白尿方面均有疗效(P0.05,P0.01)。(3)中西医结合、中医治疗均能降低血沉(P0.01,P0.05)。(4)中西医结合治疗及中医治疗均能降低dsDNA(P0.05)。中西医结合治疗能升高C4(P0.05)。结论:滋阴清热、补肾化瘀中药联合CTX能改善蛋白尿、调节免疫的作用,减少激素用量。

Clinical Research on Lupus Nephritis by the Treatment of TCM Combined with CTX

Objective：To observe curative effect of TCM combined with CTX for lupus nephritis.The TCM can Nourish yin and clear away heat,Nourish kidney melt stagnation.Methods：It was a randomized,controlled trial for three years.The study was undergone in the rheumatism Department of Shanghai Hospital of TCM.Form 2003 to 2006 total 60 in-patients and out-patients with SLE were enrolled divided into two groups.There were 30 patients in every group.One group was treated with TCM,the other was treated with TCM combined with cyclophosphamide.At 6 months after treatment,we observed the symptoms and relative indices.Results：（1）The total effective rate was 96.7% in the TCM group,93.33% in the TCM combined with cyclophosphamide group.（2）Both group could improve total albuminuria（P0.05,P0.01）.（3）Both group could declining ESR（P0.01,P0.05）.（4）The antibody of dsDNA was decended after treatment of TCM combined with cyclophosphamide and TCM（P0.05）.C4 was ascended after treatment of TCM combined with cyclophosphamide（P0.05）.Conclusion：It was concluded that TCM for nourishing yin and clearing heat could effectively improve albuminuria and regulate the immunologic function.It was also confirmed that the TCM combined CTX could reduce hormone amount used.
