摘    要:目的 探讨慢性肾功能损伤的实验室预警指标及与肾功能损伤的相关性.方法 将我院收治的127例慢性肾功能损伤患者作为研究组,根据患者肾小球滤过率(GFR)及尿微量白蛋白(mAlb)情况,将127例慢性肾功能损伤患者分为轻、中、重三组,同期选择30例来我院体检的健康者作为对照组,观察肾功能损伤患者与对照组胱抑素C(Cys-C)、血清尿素氮(BUN)、肌酐(Scr)及B型钠尿肽(BNP)等肾功能指标,并分析各项检测指标与肾功能损伤的相关性.结果 研究组患者Cys-C、β2微球蛋白(β2-MG)、Scr、BUN及BNP水平均高于对照组(P<0.05),研究组中不同肾功能损伤程度患者以上肾功能指标及Cys-C、Scr、BUN阳性检出率均有显著差异(P<0.05).Pearson相关性分析显示,血清Cys-C、Scr及BUN水平与肾功能损伤呈显著负相关,且Cys-C相关系数(r=-0.872)明显优于Scr及BUN(P< 0.05).结论 血清Cys-C、Scr及BUN水平与肾功能损伤有显著相关性,可作为肾小球滤过率异常症状早期诊断的重要指标,在慢性肾功能损伤的诊断和疗效判断中具有重要应用价值.

Abstract:Objective To investigate the chronic renal injury early warning indicators and laboratory and renal impairment correlation. Methods 127 cases of patients with chronic renal injury admitted to our hospital as a research group, based on the patient glomerular filtration rate (GFR) and urinary albumin (mAlb) cases, 127 cases of chronic renal injury were divided into light , moderate and severe groups, over the same period 30 patients in our hospital examination of the healthy control group, patients with renal impairment was observed in the control group cystatin C (Cys- C), blood urea nitrogen (BUN), creatinine (Scr) and Btype natriuretic peptide (BNP) and other indicators of renal function, and analyze the correlation between the measured parameters and renal injury. Results The study group patients Cys- C, β2 - microglobulin (β2 - MG), Scr, BUN and BNP levels were higher(P 〈 0.05), patients with varying degrees of renal impairment study group than renal function and Cys- C, Scr, BUN positive detection rates were significantly different(P 〈 0.05). Pearson correlation analysis showed that serum Cys- C, Scr and BUN levels and renal function was significantly negatively correlated, and Cys- C correlation coefficient( r = -0. 872) was significantly better than Scr and BUN( P 〈 0.05). Conclusion The serum Cys- C, Scr and BUN levels and renal function were significantly correlated, as glomerular filtration rate abnormalities important indicator of early diagnosis has important applications in the diagnosis and therapeutic evaluation of chronic renal injury value.
