进展性右上叶肺癌的外科治疗

报告了作者采用６种不同术式的气管支气管成形术治疗进展性右上叶肺癌８７例的疗效，并探讨其适应证、手术及麻醉经验，并发症防防治等。统计资料示本组病人术后１、３、５年生存率分别为８４．７％、５７．４％、３５％。可见采用不同术式气管支气管成形术治疗进展性右上叶肺癌有其明显临床实用性，值得推广。

Surgical Treatment of Advanced Carcinoma of the Right Upper Lung

This paper reported the surgical treatment of 87 cases of advanced carcinoma of the right upper lung with various tracheobronchoplastic procedures between 1979-1992. The procedure modalities included:(1) right upper lobectomy with right main bronchus wedge resection in 16 cases, (2) right upper sleeve lobectomy in 45 cases,(3) right upper lobectomy and tracheobronchoplastic procedure for the lateral wall resection in the lower trachea with/without the intermediate bronchus in 1 case,(4) right upper lobectomy with carinal resection and reconstruction in 3 cases, (5) right sleeve pneumonectomy in 13 , and(6) right pneumonectomy with tracheobronchoplastic procedure for the lateral wall resection of the lower trachea in 9 cases. The 1-, 3-, 5-year postoperative survival rates were 84.7%, 57.4% and 35% resPectively. It is concluded that these surgical Procedures are valuable.
