经根管使用派丽奥治疗牙周牙髓联合病变的临床体会

目的 观察盐酸米诺环素(派丽奥)经根管给药对牙周牙髓联合病变的临床疗效.方法 选择行牙周基础治疗(即龈上洁治、龈下刮治、根面平整等)后,牙龈无明显红肿,牙周无明显溢脓,但仍有Ⅱ度松动的牙周牙髓联合病变的患牙169颗,按根管预备时间随机交替分为两组.实验组采用派丽奥经根管途径及牙周袋同时对患牙进行治疗；对照组则采用常规樟脑酚(CP)棉消毒根管,同时牙周袋内注入派丽奥对患牙进行治疗.分别在两组患牙根管封药后2w、4 w和3个月进行疗效评价.结果 根管封药后2 w、4w,实验组牙周探诊深度(PD)、松动度指标好于对照组(P＜0.05)；3个月后,实验组临床疗效优于对照组(P＜0.05).结论 派丽奥根管给药对牙周牙髓联合病变的疗效优于CP消毒根管.

Treatment of combined periodontal-endodontic lesions by administration of minocycline hydrochloride via root canal

Objective To observe the clinical curative effects of minocycline hydrochloride via root canal on combined periodontal-endodontic lesions.Methods After receiving the periodental non-surgical treatment(i.e.supragingival scaling,subgingival scaling,and root planning,etc),patients(169 teeth) without obvious red swelling in gums and obvious periodontal pyorrhea but with grade Ⅱ loosening of combined periodontal-endodontic lesions were selected and randomly and alternately divided into two groups according to the root canal preparation time.Experimental group received the treatment on the bad teeth with minocycline hydrochloride both via the root canal and periodontal pockets.Control group received the disinfection of root canal by conventional phenocamphor(CP) cotton and the injection of minocycline hydrochloride into the periodontal pockets at the same time.Curative effect evaluation was made 2,4 w,and 3 months after the root dressing of the two groups,respectively.Results The periodontal probing depth(PD) and the degree of loosening in the experimental group were better than those indexes in the control group 2 and 4 w after the root dressing(P<0.05).The clinical curative effects in the experimental group were superior to those in the control group 3 months after the root dressing(P<0.05).Conclusion For the treatment of combined periodontal-endodontic lesions,the curative effects of administration of minocycline hydrochloride via root canal are superior to those of root canal disinfection by CP.
