等离子PK刀治疗犬脑出血疗效的实验研究

摘    要:目的探讨等离子PK刀对实验性脑出血的治疗作用。方法以钩破大脑中动脉的方法建立脑出血模型。成立建立脑出血模型的实验犬被随机分为等离子刀组、双极电凝组和对照组,通过比较三组动物之间的血肿清除情况及行为学评分,以及比较等离子刀组与双极电凝组的手术时间及术中出血量,对PK刀的止血效果进行评估。结果脑出血后等离子刀组和双极电凝组的血肿清除情况均优于对照组(P<0.05);等离子刀组和双极电凝组的神经功能恢复优于对照组(P<0.05),等离子组与双极电凝组差异无显著者性(P>0.05);与双极电凝组相比,等离子刀组的手术时间明显缩短,术中出血量显著减少。结论等离子PK刀在脑出血血肿清除术中具有一定的优势,具有止血效果佳、热副射损伤范围小、操作简单等优点,值得推广应用。

The effect of plasmakinetic tissue management system on dogs with cerebral hemorrhage

Abstract:Objective To observe the effect of plasmakinetic Tissue Management System(PK scalpel) on dogs with cerebral hemorrhage. Methods 24 dogs were used to establish the model of cerebral hemorrhage by perforating the mid-artery of brain. CT and dissection were used to observe the hemorrhage. The 18 dogs with cerebral hemorrhage were randomly divided into PK scalpel group, bipolar coagulation group and control group. After 6 hours, hematoma was evacuated from cerebral parenchyma by use of ventriculoscope, with plasmakinetic scalpel or bipolar coagulation scalpel, in PK scalpel group and bipolar coagulation group. The effect of plasmakinetic scalpel was evaluated by observation of the hemorrhagic clearance with CT scan and operating time, blood loss, as well as ethology score, among PK scalper group, bipolar coagulation group and control group. Results Brain hemotoma of PK scalpel group and bipolar coagulation group was smaller than that of control group after ICH(P<0.05). Neuro-functional recovery of PK scalpet group and bipolar coagulation group was better than that of control's on third day and seventh day after ICH(P<0.05), and there was no significant difference between PK scalpel group and bipolar coagulation group(p>0.05). And compared to dipolar coagulation scalpel, PK scalpel group showed less bleeding and less operation time(P<0.01). Conclusion Preparation of dog model of ICH by perforating the mid-artery of brain is stable and reliable. Plasmakinetic scalpel provides good hemostasis with heat radiation effect in small space; and is easy to manipulate, so that worth promoting in ICH.
