非窥镜微创低位小切口入路行甲状腺手术2336例分析

目的探讨非窥镜微创低住小切口入路行甲状腺手术方法。方法1998～2006年1月我院对2336例甲状腺患者采用低位小切口入路行甲状腺手术，对传统甲状腺手术入路进行改进。结果本组2336例甲状腺患者，切口选择在胸骨上1．5cm，长度3～4cm，取低位小切口入路经峡部径路行甲状腺手术，手术时间平均50min，术中出血量平均为50耐。术后喉返神经损伤46例（均为单侧），5例术后出血，发生于术后2～4h，出血量200～400ml，行手术止血。138例术后出现短暂性低钙血症，术后未见甲状腺功能低下。结论低位小切口甲状腺手术可达到常规手术切口切除范围。手术时间缩短。并发症与传统手术方法差异无统计学意义，而且美学效果好。

Study on thyroidectomy through low-set mini -incision approach in 2 336 cases

Objective To explore the feasibility and clinical advantages of thyroidectomy through low - set mini - incision approach. Methods 2 336 patients underwent thyroidectomy through low - set mini - incision approach in this hospital during January 1998 to January 2006 were studied. The results of this kind procedures were compared with those of traditional methods. Results The thyroidectomy was successfully accomplished in all 2 336 cases. The mini - incision was located at 1.5 cm above the sternum with a length of 3 cm to 4 cm. The mean operation time was 50 rain. Blcod loss during the operation was 50 nil. 46 cases had hoarseness for short time, 138 cases had temporary hypocalcemia and 5 cases had postoperative bleeding. There was no hypothyroidism occurred. Condusion The thyroidectomy through low - set mini - incision approach is safe and applicable with good cosmetic effect.
