颞肌筋膜瓣即刻修复上颌骨缺损

目的 通过对颞肌筋膜瓣即刻修复上颌合缺损病例的回顾性分析，评价其效果。方法 本文总结了22例上颌骨切除后用颞肌筋膜瓣即刻修复的临床经验。其中采用单纯颞肌筋膜瓣修复16例，采用颜肌筋膜瓣—喙突复合移植者6例。结果 22例组织瓣全部成活，未出现明显炎症，语言及咀嚼功能恢复良好。合并有眶底、眶下缘骨质缺损用喙突—颞肌筋膜瓣修复者，未出现眼球内陷，视力正常。面中l／3形态基本正常，未出现明显畸形，但有3例出现轻度险外翻。结论 颞肌筋膜瓣即刻修复上颌骨安全可靠，功能和形态恢复良好；颞肌筋膜瓣—喙突复合移植者可以避免和减少因眶底缺损而引起的眼科并发症的发生。

Immediate reconstruction of maxillectomy defects using temparolis myofascial flap

Objective To assess the effect of immediate reconstruction of maxillectomy defects by using temparolis myofascial flap. Methods A retrospective study was made of 22 patients who underwent immediate reconstruction surgery using temparolis myofascial flap following maxillectomy since 1989. sixteen cases were repaired by temparolis myofascial flap alone, 6 cases by combined corone - temparolis myofascial flap. Results All flaps were alive without obvious inflammation,speech and chewing function were normal. There were no cases with enophthalmos in 6 cases constructed by combined corone- temparolis myofascial flap,all patients had adequate functional vision. No deformity occurred,but 3 out of the 6 cases were with mild. Conclusion Temparolis myofascial flap and corone - temparolis myofascial flap were respectively safe, reliable,and effective for reconstruction of maxillectomy defects with or without orbital floor defect.
