幽门螺杆菌阳性非溃疡性消化不良疗效观察

目的幽门螺杆菌(HP)感染是非溃疡性消化不良(NUD)的常见诱因。我们研究通过清除HP感染后,观察患者的临床症状改变和预后转归情况。方法经胃镜和病理组织学检查,HP(+)的NUD患者57例,所有患者均接受抗HP治疗,包括甲硝唑、羟氨苄青霉素和枸橼酸铋剂。经1周治疗后,胃镜检查HP清除情况,并回顾分析经HP清除后,患者的临床症状变化和预后转归。同时又进行3个月的随访,观察其症状变化。结果经1周抗HP治疗,发现HP转阴的34例NUD患者的临床症状,如上腹疼、进食后上腹部不适、暧气、恶心和呕吐均有明显改善(P<0.05)。而HP仍持续阳性患者23例的临床症状无明显变化(P>0.05)。经过3个月随访发现,HP转阴的34例NUD患者的临床症状仍然保持良好的状态,且与治疗前比较仍有明显好转(P<0.05)。结论HP感染清除后,可有效地改善NUD患者的临床症状。因此,临床上对于HP阳性的NUD患者在使用抑酸剂的同时,可结合抗HP治疗来提高疗效。

Effect of Eradication of Helicobacter Pylori on Prognosis of HP - positive Non - ulcer Dyspepsia

Objective Helicobacter pylori (HP)infection is one of the most common pathogens in the induction of non-ulcer dyspepsia (NUD).In this study,we investigated the effect of eradication of HP on the prognosis in patients with HP-positive NUD.Methods 57 patients with HP-positive NUD were recruited in our study,and routinely undertaken eradication therapy against HP,including metronidazone,amoxicillin and bismuth sybnitrate.A week after anti-HP treatment,clinical manifestations were retrospectively analyzed.Results A week after anti-HP therapy,clinical manifestations were significantly improved in 34 NUD patients with HP eradication(P< 0.05). In contrast,there were no marked changes in 23 NUD patients who still had HP-positive reaction(P>0.05).After 3 month follow-up,all clinical symptoms in 34 HP-negative NUD patients were still maintained improvement and demonstrated markedly relieved (P<0.05).Conclusion Eradication therapy of HP infection could effectively allievate clinical symptoms in patients with HP-positive NUD.Therefore,a combination of anti-HP treatment with acid-suppressing drugs may have a benefit for these patients.
