亚低温联合脑室外引流加尿激酶灌洗治疗高血压脑室内出血的临床研究

目的探讨亚低温联合脑室外引流加尿激酶灌洗治疗高血压脑室内出血的临床疗效。方法选择108例高血压脑室内出血患者,随机分成亚低温治疗组和常温对照组,其中常规组给予双侧侧脑室外引流加尿激酶灌洗,亚低温治疗组除上述治疗外,给予亚低温治疗48～72 h。结果术后第1天及第3天亚低温组患者颅内压低于对照组,差异有统计学意义(P<0.05)。亚低温组术后3个月ADL分级法生存质量良好率明显高于对照组(P<0.05)。结论亚低温治疗能够有效降低高血压脑室内出血患者的颅内压,减轻脑水肿,容易控制中枢性高热,有效降低患者的病死率,提高其生存质量。

Clinical Study of Treatments of Hypertensive Intraventricular Hemorrhage by Mild Hypothermia And Ventricular Drainage with Urokinase Irrigation

Objective To investigate clinical effect of treatments of hypertensive intraventrieular hemorrhage by mild hypothermia anti ventrieular drainage with urokinase irrigation. Methods 108 eases of intraventrieular hemorrhage in hypertensive patients were randomly divided into room temperature treatment group and the mild hypothermia treatment group,in room temperature treatment group giving both lateral ventricular drainage and urokinase irrigation,in mild hypothermia treatment group, in addition to the above treatments,giving mild hypothermia treatment for 48-72h. Results The intraeranial pressure of the patients in mild hypothermia-treated group was significantly lower than the control group in 1 and 3 days alter operation（ P 〈 0.05 ） , good rate of survival quality after 3 months of mild hypothermia group was significantly higher（ P 〈 0.05 ）. Conclusion Mild hypothermia treatment can effectively reduce intracranial pressure of hypertensive patients with intraventriealar hemorrhage,and reduce cerebral edema,easily eontrol eentral heat and effeetively reduce mortality aml improve the quality of life of patients.
