实时三维超声心动图对高血压病患者左心室壁节段性舒张功能的研究

目的应用实时三维超声心动图（RT-3DE）评价高血压病患者左心室壁节段舒张功能。方法20例高血压患者和20例健康者（对照组），RT-3DE测量其左室壁17节段每搏量、舒张末容积、节段充盈率。结果40例受检者均获得了具有清晰内膜边界的实时三维图像和17节段容积-时间曲线，高血压病组曲线排列紊乱，各节段达收缩、舒张末最小容积的时间点差别较大；高血压病患者左室17节段中前壁中间段、下壁心尖段的节段每搏量高于对照组（P〈0．05），中前侧壁基底段、前侧壁中间段、下壁心尖段、侧壁心尖段的节段舒张末期容积高于对照组（P〈0．05），中后壁基底段、侧壁基底段、前壁中间段、前间隔中间段、后间隔中间段、下壁中间段、前侧壁中间段、前壁心尖段、下壁心尖段、侧壁心尖段的节段充盈率低于对照组（P〈0．05）。结论高血压病患者与正常人左室壁节段舒张功能存在差异，RT-3DE是一种可行的、准确定量评价左室壁节段舒张功能的新技术。

Study of segmental diastolic function of left ventricular wall in patients with hypertension by real-time three-dimensional echocardiography

Objective To assess the segmental diastolic function of left ventricular wall in patients with hypertension by real -time three -dimensional eehoeardiography （RT -3DE）. Methods Twenty patients with hypertension and 20 healthy subjects were enrolled in this study. The 17-segment myocardial model was adopted. The regional stroke volume（rSV） ,regional end-diastolic volume （rEDV） and the regional peak filling rate （rPFR） were assessed by RT - 3DE. Results RT - 3DE imaging with clear endomembrane bouneary and 17 segments volume - time curve of 40 subjects were obtained. The curve was disordered in hypertension group, there was difference of minimum volume time in end-systole and end-diastole of each segment （P 〈 0. 05 ）. Conclusion There is difference of segmental diastolic function of left ventricular wall between patients with hypertension and normal subjects. RT - 3DE is a new and feasible method in assessing the segmental diastolic function of left ventricular wall accurately.
