充血性心力衰竭预后的高危因素分析

目的　探讨影响充血性心力衰竭 (CHF)预后的高危因素及其与预后的相关性。方法　选择资料较完整的CHF病例 72例。观察其血压、空腹血糖、口服葡萄糖耐量试验 (OGTT)、血脂、心电图和应用彩色多普勒测量的左室内径 (LVD)、左室后壁厚度 (LVPW )、室间隔厚度 (IVS)、左室射血分数 (LVEF)等指标的变化。结果　CHF存活组与死亡组比较 ,年龄、心衰病程、LVEF、LVD、室性心律失常、糖尿病 (或糖耐量异常 )等均有显著性差异 (P <0 .0 1～ 0 .0 5 )。结论　CHF患者年龄、心衰病程、LVEF、LVD及室性心律失常、糖尿病是影响其预后的高危因素。

Analysis of high risk factors in prognosis of congestive heart failure

Objective To analyze the high risk factors in the prognosis of congestive heart failure(CHF)and to discuss the correlation between the high risk factors and the prognosis.Methods Blood pressure,fasting blood glucose,oral glucose tolerance test(OGTT),serum lipid,and ECG were observed in 72 CHF cases.Left ventricular diameter(LVD),left ventricular posterior wall(LVPW),interventricular septum(IVS),and left ventricular ejection fraction(LVEF) were measured by color Doppler.Results There were significant differences in age,course of heart failure,LVEF,LVD,ventricular arrhythmia and diabetes mellitus(or impaired glucose tolerance) in CHF survival group and CHF death group.(P<0.01-0.05).Conclusion Age,course of heart failure,LVEF,LVD, ventricular arrhythmia,and diabetes mellitus are high risk factors in the prognosis of CHF.
