| 摘 要： | 目的探讨内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)治疗直肠类癌的临床疗效及安全性。方法选择7例结肠镜发现的距肛门8 cm以内直肠肿物行ESD治疗。内镜下先于病变边缘做标记点,沿标记点环形切开黏膜,于病灶下方剥离黏膜下层,同时多次黏膜下注射,保持病灶与肌层分离,直至完整剥离肿瘤。结果 7例术后病理诊断为直肠类癌,肿瘤直径0.9~1.6 cm,均成功经内镜切除,基底及距切缘0.2 cm以内无类癌组织累及。随访13~28个月,未见肿瘤残留和复发。结论 ESD用于治疗直径≤2.0 cm、未侵犯固有肌层的直肠类癌,具有较好的安全性。
