胰腺实性假乳头状瘤CT及MRI所见分析

目的探讨胰腺实性假乳头状瘤(SPTP)的CT及MRI表现。方法回顾性分析经手术病理证实的SPTP28例。术前常规CT平扫及增强检查17例,MR平扫及增强检查11例,其中3例同时CT及MR检查。结果检查发现病灶位于胰头10例,5例位于胰颈,7例位于胰体,6例位于胰尾部,肿瘤直径2.2～11.3cm。27例具有完整包膜;1例包膜不完整。20例呈囊实性表现,3例以囊性成分为主,5例以实性成分为主。8例可见钙化。4例病灶内可见出血。25例病灶的实性成分动脉期CT值增加(22.1±4.3)HU,门静脉期CT值增加(37.7±5.8)HU,囊变区无强化。结论正确认识CT及MR的影像学特点,可以对绝大多数SPTP作出术前诊断。

CT and MRI findings of solid-pseudopapillary tumor of pancreas:A retrospective analysis of 28 cases

Objective To investigate the CT and MRI features of solid-pseudopapilary tumor of the pancreas (SPTP). Methods SPTP confirmed by pathology in 28 cases was retrospectively analyzed, among whom 17 cases underwent plain and enhanced CT scan; 11 cases, plain and enhanced MR scan, 3 cases, CT and MR scan. Results Ten cases were located in pancreatic head, 5 in pancreatic neck, 7 in pancreatic body and 6 in pancreatic tail. The diameters of tumors were 2.2-11.3 cm; 27were encapsulated, 1 was not encapsulated; 20 cases were cystic-solid mixed; 3 lesions appeared; 5cases appeared solid; 8 cases with calcification; 4 cases with hemorrhage. The CT value of the solid lesion increased (22.1±4.3) HU in arterial phase and increased (37.7±5.8) HU in portal phase, the cystic part of all lesions showed no enhancement. Conclusion Correct recogniting CT and MR imaging features can diagnose most SPTP at preoperation.
