胆管细胞癌的CT、MRI表现

目的提高对胆管细胞癌CT和MRI表现的认识。方法回顾性分析54例经病理证实的胆管细胞癌患者的CT和MRI表现。结果34例肿块型胆管细胞癌,表现为边界清楚的长T1长T2信号或低密度肿块影,动脉期周边强化和中心延迟强化。其中2例为肝硬化基础上发生的胆管细胞癌,表现为增强各期周边强化的肿块;7例管壁浸润型胆管细胞癌,表现为管壁的不规则增厚、管腔狭窄、管壁的异常强化以及远端胆管的扩张;13例管内型胆管细胞癌表现为异常强化的腔内肿块。结论胆管细胞癌的影像学表现具有一定的特征性,CT和MRI对其诊断有着重要价值。

CT and MRI Imaging of Cholangiocarcinoma

Objective To improve the recognition on CT and MRI features of cholangiocarcinoma. Mothocls 54 cases of pathologically confirmed cholangiocarcinoma were retrospectively reviewed, all patients received CT and/or MR studies （before and post contrast）, the imaging findings were analyzed. Results 34 cases of mass-forming cholangiocarcinoma, in which 32 cases showed a mass of low signal intensity on T1WI and high signal intensity on T2WI or low density on CT scan, and periphery enhancement in arterial phase and inner enhancement in delay phase. 2 cases occurred in a cirrhotic liver, demonstrated periphery enhancement in each phase of enhancement; 7 cases of periductal infiltrating cholangiocarcinoma showed irregular periduct thickening and increased enhancement with an abnormally and irregularly narrowed duct and peripheral duct dilatation; 13 cases of intraductal cholangiocarcinoma showed abnormal enhancement intraductal mass. Conclusion Cholangiocarcinoma had some typical features in CT and MRI image. CT and MRI were valuable of cholangiocarcinoma diagnosis.
