哮喘儿童痰中炎性细胞及转化生长因子-β_1表达的研究

摘    要:目的:观察哮喘儿童痰中炎性细胞及转化生长因子-β1(TGF-β1)的表达。方法:负压吸取痰标本39例,其中哮喘组(轻中度哮喘急性发作)婴幼儿24例,非哮喘组(支气管肺炎)婴幼儿15例,同时监测两组患儿的气道阻力。痰标本上清液测白介素-5(IL-5);沉淀物涂片分别行瑞氏染色作普通分类计数及免疫细胞化学测TGF-β1的表达。结果:哮喘组气道阻力为(0.53±0.26)kPa·L-1·S-1明显高于非哮喘组(0.25±0.16)kPa·L-1·S-1,P<0.01;哮喘组嗜酸性粒细胞(Eos)计数为(20.25±15.36)%、IL-5为(16.85±6.54)μg/L与非哮喘组(1.69±2.07)%、(13.56±3.06)μg/L比较,差异有显著性,分别为P<0.01、P<0.05。15例哮喘患儿痰液炎性细胞中出现TGF-β1阳性表达,阳性表达率与Eos计数、IL-5之间呈显著线性相关,分别为(r=0.780,P<0.01),(r=0.892,P<0.01),与气道阻力之间未见明显相关性;非哮喘患儿未见有TGF-β1阳性表达。结论:TGF-β1在哮喘儿童痰中有过度表达,提示TGF-β1与哮喘有关,可为哮喘的诊断提供依据。

Abstract:Objective:To study the inflammatory cells expressing of TGF-β1 in sputum of children asthma.Methods:Sputum specimens of 39 cases were obtainted by vacuum aspiration including asthma group (acute asthma): 24 cases, non-asthma group (acute pneumonia):15 cases. The airway interrupter resistance (Rint) of all cases were measured with microloop spirometry devicem. The depositor of sputum specimens was smeared for Eos count and measurement of TGF-β1 expression of inflammtory cells; and the supernatant was assessed for content of IL-5 with ELISA.Results:The mean Rint was significantly higher in asthma group (0.53±0.26) kPa/L·S than in non-asthma group (0.25±0.16) kPa/L·S (P<0.01).The Eos count and expression of IL-5 were higher in asthma group than in non-asthma group (P<0.01, P<0.05) respectively.15 cases showed positive expression of TGF-β1 of inflammatory cells in sputum of asthma group, there was no positive expression of TGF-β1 in non-asthma group. Forthermore, the ratio of TGF-β1 positive expression showed linear correlation with Eos count and IL-5 (r=0.780, P<0.01, r=0.892, P<0.01) respectively. There was no relationship with the Rint.Conclusion:The over-expression of TGF-β1 of inflammatory cells in sputum of asthma patients reflects the affection of asthma patient′s airway, there is relationship between the asthma and inflammatory cells expressing of TGF-β1, which may provide evidences for diagnosing of asthma.
